Nominate a Nurse for the DAISY Award

 About The DAISY Foundation

The DAISY Foundation was established in 2000 by the family of J. Patrick Barnes, who had died at the age of 33, from complications from auto-immune disease Idiopathic Thrombocytopenia Purpura (ITP). (DAISY is an acronym for disease attacking the immune system).  During Pat’s 8-week hospitalization, his family was awestruck by the care and compassion his nurses provided not only to Pat but to everyone in his family.  So they created the foundation in Pat’s memory to recognize extraordinary nurses everywhere who make an enormous difference in the lives of so many people by the super-human work they do every day. What is the DAISY Award?

​The DAISY Award is a nationwide program that rewards and celebrates the extraordinary clinical skill and compassionate care given by nurses every day.  This award is given monthly to an outstanding nurse in over 50 hospitals across America. Winslow Indian Health Care Center is proud to be DAISY Award Hospital Partner, recognizing one of our nurses with the special honor twice a year.

Honoree Recognition

Each DAISY Award Honoree will be recognized at a public ceremony in his/her unit and will receive:
  •  a beautiful certificate,
  •  a DAISY Award pin,​
  • a hand-carved stone sculpture entitled “A Healer’s Touch”​

Criteria for Nomination

​DAISY Award honorees personify Winslow Indian Health Care Center’s remarkable patient experience.  These nurses consistently demonstrate excellence through their clinical expertise and extraordinary compassionate care, and they are recognized as outstanding role models in our nursing community. They should demonstrate:
  • Excellent clinical nursing skills
  • Compassionate care
  • Excellent educator of patient & their families
  • Excellent interpersonal skills
  • Continued commitment to excellent

How to Nominate an Extraordinary Nurse

If you want to say Thank You by sharing your story of how a Winslow Indian Health Care Center nurse made a difference you will never forget.  Anyone may nominate a deserving nurse by filing out the DAISY Award Nomination form online by clicking or tapping the button link below or submitting the downloadable form to the DAISY boxes located either at the Greeters desk, Urgent Care, Nurses Station 1, 2, 3 or at Leupp and Dilkon Health Stations.

DAISY Award Honoree

Nomination letter excerpt:
"​I have never nominated anyone for the daisy award before.  This year has been filled with uncertainty, I believe all nurses and all healthcare professionals have done a great job with taking on the new challenges and roles that need to be done to take care of our patients.  I have worked with many nurses.  I see the struggles some are faced with every day.  Some of them have their own way with dealing with it and find a way to get the work done.  The nurse I nominate for this year’s Daisy Award, is a nurse who I have seen exemplify many great qualities a nurse should.  He is more than nurse but a friend to many patients and coworkers.
​
His name is Brian Hansen.  Brian is a great nurse. He takes on any challenge with no question. He enjoys helping his patients and making sure everything needed for their visit is taken care of.  He is always on top of everything and always keeps an open mind.  There are many things to say about Brian.  Everyone who has worked with Brian could agree that he has exceptional communication skills and has never-ending diligence.  Also his attention to detail and being action-oriented.  Most of all, I know truly cares and enjoys caring for all his patients and he always makes sure that they’re comfortable and he makes sure that they know their complete care plan.  These qualities make Brian a great nurse and we are lucky to have him here at WIHCC.  He truly demonstrate our mission, vision, and values.  Thank you for taking the time to read this."
