Winslow Indian Health Care Center offers patients a variety of diabetic services. Hózhǫ́ógo Iiná Wellness Program (HIWP) formally known as Diabetes Program provides education and support services for people living with diabetes, and their families. The program is funded by Special Diabetes Project for Indians (SDPI) grant and Winslow Indian Health Care Center, this service is provided for all Native Americans.

Diabetes Self-Management Education
DSME is a class designed for diabetic patients and their families. The class will help patients gain information, skills and abilities for diabetic self-care. The class aims to have the patient make informed decisions about self-care behaviors, problem-solving and collaborations with our excellent health care teams to improve the patient's health status and quality of life.
At Hózhǫ́ógo Iiná Wellness Program (HIWP) we encourage the families of patients to join the class, in doing so, family members will gain an understanding of what their loved ones deal with on a daily basis.

Classes are held in the Hózhǫ́ógo Iiná Wellness Center and organized by HIWP and led by Family Nurse Practitioner & Clinical Provider, Lita Scott and accompanied by her clinical staff in educating patients. DSME is held every 2nd & 4th Wednesdays of each month. Class begins at 8 am to 1 pm, to attend a class, the patient's Provider must send a referral to the Wellness Program or the Program can also reach out to a Provider for a referral.
The Class will begin with a food demonstration and will display how a patient can prepare a healthy and diabetes friendly meal. Patients and families will be also be provided free breakfast, lunch, and many, many, many incentives!

Diabetes Shoe Clinic
Shoes can be an important part to a self care process for a diabetic patient, so one unique step HIWP has done is provide shoes, socks, inserts and education through the Diabetes Shoe Clinic. These wonderful shoes are specially designed shoes or inserts referred to as extra depth, therapeutic shoes intended to reduced the skin breakdown in a diabetic with pre-existing foot disease. A referral is needed to receive a pair of shoes offered through SDPI grant at no cost to the patient. A patient is eligible to receive 1 pair of shoes a year, and receive shoe inserts up to 3 a year. Shoes clinic are available twice a month by making an appointment or a referral is made by the provider. |
Diabetes in Pregnancy
Gestational diabetes is a type of diabetes that is first seen in a pregnant woman who did not have diabetes before she was pregnant. Diabetes is a disease in which your blood glucose, or blood sugar, levels are too high. When you are pregnant, high blood sugar levels can be harmful for your baby. Regular visits with members of a health care team who are experts in diabetes and pregnancy will ensure that you and your baby get the best care. Your health care team include: a medical doctor, obstetrician, diabetes educator, nurse practitioner, registered dietitian. You are are the most important team member of the team. Your health care team can give you expert advice so you can have a health baby. Breastfeeding support and Breastfeeding Hotline are available. |

Health and Fitness
HIWP is proud and pleased to have an out reach service that offers a very diverse Health & Fitness program and community activities to suit just about anybody. Whether you are an experienced fitness pro or not, our staff are trained to work with you at your pace and comfort level. Participate in community fitness events like Just Move It, Turkey Trot, or come over to the Wellness Center and pick from many Group Fitness classes offered daily like Cardio Kick, Chair Yoga, Functional Training, Mom's in Motion, and Zumba, Tae Kwon Do. These are just a few of the many Group Fitness class available, we encourage you to stop by pick up a Group Fitness calendar or talk to one of our fitness techs. |
Nutrition
Our Nutrition service plays an important part to the HIWP by coordinating wellness activities, one on one consultation, group prevention education, group prenatal nutrition, home glucose meter education and many more. Through the program you will receive in depth education on proper food choices and food preparation for a health and healing lifestyle. |
Youth Wellness
HIWP coordinates with Northern Arizona Health Care in presenting Fit Kids. A program that utilizes the C.A.T.C.H. model, an acronym for Coordinated Approach to Child Health. This model coordinates a team of health professionals that involves Providers, Nurse Practitioners, Behavioral Health, Physiologist, Dietitians, Health Fitness Coordinator to collaboratively create a community of health. Youth up to 18 years of age with a body mass index that is elevated or other conditions determined by a provider, is eligible to participate in the program. Fits kids runs twice a month and during that time, they will have fun with physical activity and learn behavioral changes in food choices. |
Navajo Traditional Medicine Program
Founded on the principle that the best health and wellness occurs when there is balance between the spiritual, mental, emotional, and physical parts of a person. We strive to integrate traditional Dine and Native American spiritual healing with allopathic/osteopathic modalities in its delivery of care where appropriate, necessary, and desired by the patient. It encompasses a wide array of healing practices that are based upon the indigenous knowledge of the Dine and Native people. |
