Patient Benefits Coordinator

Veterans, Need Help with VA Benefit?

​We will have a Veteran’s Representative from Congressman Tom O’Halleran’s office at the Winslow campus Medical Office Builing (MOB) on  Wednesday, December 18th from 12:00 pm to 2:00 pm.    The representative will be set up in the MOB front lobby and can help any Veteran having difficulties with their VA benefits.  Please spread the word to our Vets!

PBC will also set up a table, if patients have questions regarding their benefits.

Patient Benefits Coordinator

The Patient Benefits Coordinator is the advocate for the patient in the search of and utilization for alternate resources such as AHCCCS, Medicare, Medicaid, Department of Veterans Affairs (VA), Private Insurance Billing, etc. and helping with their enrollment. 

Our Patient Benefits Coordinator meets patients daily and knows firsthand that educating the individual about their health care choices is key to building healthy families and healthy communities.  Our goal is to ensure that all patients who are eligible for alternate resources are identified, contacted and encouraged to apply for and maintain eligibility for available benefits.
Just a Reminder.....
If you have any medical bill or referral pending; patients are required to either apply for AHCCCS or complete an AHCCCS Screening every 6 months before Purchase Referred Care (PRC), formerly known as Contract Health, payment can be authorized or appointment can be scheduled.
Medicare cost sharing program
Can provide help with Medicare premiums and/or medical coverage for people who are age 65 or older, blind or disabled.  You be eligible for one of the three programs:
  • Qualified Medicare Beneficiary (QMB):  Individual $1,041;  Couples $1,410.
  • Specified Low-Income Beneficiary (SLMB):  Individual $1,041.01 - $1,249.00;  Couple $1,410.01 - $1691.00
  • Qualified Individual (QI-1):  Individual $1,249.01 - $1,406.00;  Couple $1,691.01 - $1,903.00 
medicare enrollment
  • Part B:  Starts January 1 to March 31 of each year.
  • Part D:  Starts October 15 to December 7 of each year.

social security benefits
You can apply for early retirement benefits at age 62 or until apply at the full retirement age. We can also help you apply using Social Security Administration's online services or make an appointment for you to visit the local Social Security Office.
Social security disability
For adults/children with chronic illness or disease, you maybe eligible for Disability.  If you are approved, you can receive monthly benefits and you may qualify for other health care programs such as Medicare or Medicaid.  We can assist you to apply for benefits online or schedule an appointment for you to visit the local Social Security Office.  
ahcccs
Arizona Health Care Cost Containment System (AHCCCS) for adults and children with limited income.
AHCCCS Eligibility Income Guidelines Effective 02/01/2019 for Adults:
  • Household 1:  $1,385.00
  • Household 2:  $1,875.00
  • Household 3:  $2,365.00
  • Household 4:  $2,854.00  (add $490 per additional person). 
kidscare
If the child's income is too high for AHCCCS Health Insurance, the child may qualify for KidsCare at a low monthly premium.  Per federal law, Native American enrolled with a federally recognized tribe, do not have to pay a PREMIUM.  If you have questions, please see a PBC representative.
WHO IS ELIGIBLE:
  • Is an Arizona resident.
  • Is 18 years old or younger.
  • Is a United States citizen or a qualified immigrant.
  • Has a Social Security number or applies for one:  1.  To apply for a Social Security number visit U.S. Social Security Administration.  2.  Make a copy of the form submitted as confirmation of applying for a Social Security Number.
  • Is under the income limit.
  • Is not currently covered by other health insurance.
  • Does not qualify for coverage through a state agency employee.
  • Is not eligible to receive AHCCCS (Medicaid) coverage.
  • Is a member of a household that is willing to pay a premium.
health insurance marketplace
Health Insurance Market place helps uninsured people find health coverage.  If you are a member of a federal recognized tribe, you can apply for Insurance through the Marketplace ANYTIME.  You may qualify for premium tax credit which may be taken as advance payments to lower monthly  premium costs, or as a refundable credit on tax return. 
Arizona breast & cervical cancer treatment program (BCCTP)
If you are uninsured and have been diagnosed with breast or cervical cancer or pre-cancerous cervical lesion, you may be able to received treatment through the BCCTP.  To qualify for the BCCTP; come see a PBC representative to check on eligibility requirements.

You may be eligible for medical benefits.  Bring the following information, so we may assist you in applying for benefits online:
  • Personal Information 
  • Spouse/Dependent Information
  • Insurance Information
  • Military Service Information (DD 214)
  • Financial Information

Arizona long term care system (altcs)
ALTCS program is for individuals who are 65 or older, blind, or disabled and need ongoing services at a nursing facility level care.  However, program participants do not have to reside in a nursing home.  Many ALTCS participants live in their own home or an assisted living facility and receive needed in-home services.
