Patient transportation is not an automatically scheduled service. Patients must call Carolyn Tso, Non-emergent Transportation Coordinator to schedule transportation three days in advance of appointment. 
Please have the following information ready when you call:

1.  Your full name,
2.  Current address 
3.  Phone number,
4.  Date of appointment, 
5.  Type of appointment (doctor,
      dentist, therapy, etc.)

Our Vehicles
HANDICAP FRIENDLY
To meet the special needs of elderly individuals and individuals with disabilities, our vehicles are equipped with a wheelchair lift and a ramp that is compliant with ADA and NHTSA standards and specifications.  This will ensure patients are safely transported.

QUALITY:
Vehicles are equipped with state of the art technology to safely and reliably transport patients in a professional and respectful manner.

COST EFFECTIVE: 
Transportation to and from Winslow Indian Health Care will be provided on a regularly scheduled basis at no cost to the patient.

ACCESSIBILITY
All patients with a verified appointment are transported, however, priority will be given to elderly and disabled patients and their caretakers. 
  

Procedure for transportation service

Transport is provide to patients to and from WIHCC clinics during the business hours of 7:30 am to 4:30 pm.
  • Patients must have a verified appointment at the WIHCC, Dilkon or Leupp clinics.
  • Must call 928-289-6281 at least three (3) days before the scheduled appointment to make arrangements.​
  • Please have appointment information available when calling the Transportation Program.​
Arrange transportation for patients who have a referral outside of the WIHCC service area. 
  • Patient must have coverage under American Indian AHCCCS prior to arranging transportation.
  • Patient must have a verified appointment and referral from a WIHCC provider.
  • Patient must call at least three (3) days before the scheduled appointment to make transportation arrangements.
  • Please have appointment information available when calling the Transportation Program.
  • The WIHCC Transportation Program is not associated with outside transportation companies and is not responsible for their services provided.
RULES FOR RIDERSHIP
1.  Seat belts must be worn at all times.
2.  No standing or moving in aisle when van is in  motion.
3.  Patients exhibiting hazardous circumstances or high risk for falls will not be permitted to ride.
4.  Van may not go to field clinics during inclement weather.
5.  Priority will be given to elderly and disabled patients (including their caretakers).
      a. Elderly patient are considered 60 years and older.
      b. Disabled patients are considered to have a physical or physical impairment which substantially limits one or more major life activities.
6.  Only one (1) escort per patient.
7.  Arrange for and have caretaker be available and ready to pick up the patient at the end of transport 
services.  ​​
