Division of Nursing
We recognizes and appreciates the diversity of our community and our nurses. Realizing the value in the ranges of experience and background, we welcome nursing interest from all areas. WIHCC Nursing also supports continuing education for our nursing staff in the pursuit of professional growth and excellence. Educational opportunities are intended to develop, refine or enhance nursing expertise.
In 2013 WIHCC Division of Nursing implemented the Patient Centered Medical Home (PCMH) model for the primary purpose of: (1) improve continuity of care for patients and providers; (2) improve appointment show rates; (3) improve patient-provider relationships; (4) improve patient outcomes.
The WIHCC PCMH program is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC). The program consists of a PCMH Manager and several Clinical Care Coordinators whom focus on coordinating all necessary information for the patient encounter to ensure the most efficient visit possible. It is also to help the patient achieve a safer and better transition from discharge to home and back into primary care for necessary follow up.
